Did you use the art generator from the android website? Device Frame Generator doesn't seem to have the nexus 6 frame yet...

Its not a theme its miui rom
Its the MIUI Rom with their launcher and a theme on top of that.

Btw the widget is included in the Rom and its called "Fancy widget" I'm not sure you can get it somewhere else.

Isn't it the default wallpaper in v4? Download a miui v4 ROM and pull it from there.
